When attending the first class, there will be hard copies of a waiver with the following info. to review and sign:
Participation Consent: I give permission for my child to participate in classes, workshops, camps, events, and enrichment experiences offered by Chrysalis Craft Club. Activities may include sensory play, arts & crafts, movement & music, fine motor exploration, seasonal themed activities, or collaborative play experiences.
Assumption of Risk: I understand that children’s activities may involve normal childhood risks including slips, falls, bumps, contact with materials, or minor accidental injury. I voluntarily assume these ordinary risks on behalf of my child.
Liability Release: To the fullest extent permitted by law, I release and hold harmless Chrysalis Craft Club, its owner, staff, assistants, and volunteers from claims arising from participation, except in cases of gross negligence or intentional misconduct.
Wellness Policy: I agree to keep my child home if they are experiencing fever, vomiting, diarrhea, contagious illness, or symptoms that may place others at risk.
Behavior & Safety Expectations: To maintain a peaceful and safe environment for all families, Chrysalis Craft Club reserves the right to dismiss participation when behavior creates an unsafe or disruptive setting.
Photo/Media Consent: ☐ YES — I grant permission for photos/videos of my child to be used for website, social media, and promotional materials. ☐NO—I do not grant permission.
Please also note the following:
-You may request a refund until 1 week prior to the start of a 4-week session or drop-in class. Beyond that, classes can not be refunded or made-up due to a participant’s absence. If the teacher is ill or has a family emergency that requires a cancellation, a full refund will be provided.
-While there won’t be much time for snacks with all of the activities, please note that the studio is a nut-free space.
-To keep messes limited to our little creations and explorations, if able, there will be a space to remove and place shoes upon entry, as well as a sink to wash hands, if needed. If you’d like your child to wear grippy socks to avoid slipping, please do. The studio can become chilly at times, so wearing layers is never a bad idea. Feel free to bring water bottles to stay hydrated. No juice or sugary drinks, please!
-There is a refrigerator available for use if needed, a bathroom, and a small space away from the activities for nursing, etc.
